Physician: MD, PhD, or DO
Fee includes badge, printed program, breakfast and breaks daily, lunch within optional educational workshops, and entrance to all educational sessions, optional workshops, the exhibit hall, the Networking & Welcome, and the Networking Dinner. Food and beverage will be served in the exhibit hall during all session breaks.

Resident/Fellow
MD or DO who is in a residency or fellowship program. Letter of verification with dates of residency is required.
Fee includes badge, printed program, breakfast and breaks daily, lunch within optional educational workshops, and entrance to all educational sessions, optional workshops, the exhibit hall, the Networking & Welcome, and the Networking Dinner. Food and beverage will be served in the exhibit hall during all session breaks.

Military MD, DO
This registration category is open to members of the US military. In order to obtain this rate, registrant must have an active military email address. Please ensure your military email address is used for your registration record to avoid any adjustments in fees.

Optometrist
All optometrists (ODs) must complete the following statement during online registration or on the registration form before registration can be processed.

All guests who wish to take part in activities associated with the meeting must be registered.

A registered attendee's spouse, child over 17 or guest who is not a physician, office administrator, ophthalmic medical personnel, or corporate personnel plus up to 3 children aged 4-17. Children under 4 will not be charged a fee. This is a non-refundable fee.

Payment
Payment (in U.S. funds) can be made by personal check, bank draft, travelers check, AmEx, VISA, or MasterCard. No other credit cards will be accepted. Mailed/faxed registrations will be accepted through January 13, 2011; online registrations will be accepted through January 21, 2011. (You must register on site after this date.)

Cancellation/Refund Policy
All cancellations and requests for refunds must be in writing and received no later than December 30, 2010. A handling fee of $125 will be deducted from each cancelled registration. NO REFUNDS will be given after December 30, 2010

 

ASCRS
